2. Describe one other regular commute, such as going from home to your child's school or from home to a lesson teacher.

3. Describe the roads you took on a vacation, such as a weekend skiing trip or use of a rental car while travelling.

1. What is your favorite meal to cook or order at a restaurant?

2. If you cook, how do you prepare this meal? Provide as much detail as possible. If you eat out, why do you choose to order this?

3. What values do you have with choosing what you eat? Do you eat what your mother cooked or what is healthiest or what is easy to cook--how do you choose what to eat?


Doctor Visit


1. Describe your medical history: past doctor visits, illneses or any regular problems such as poor digestion.

2. Do you get regular checkups? Why or why not?

3. What form of medicine do you prefer: western style or alternative medicine (acupuncture, naturapathy, etc.)?  Why?
